Required documents include a copy of the service members DD214 and the discharge must be Honorable, current photo ID, verification of address, a copy of the front and back of page one of your most recent federal tax return, most recent bank statement and copies of debts you want considered for payment. If you are disabled we need a copy of your disability compensation letter from the Veterans Administration. If you are active duty we need a copy of your current service record.

All required documents must be submitted within 2 weeks of the original request. Your application will not be considered or sent to the committee until all documents have been received in readable condition. No application will be reviewed without all appropriate documentation.
All applications go through an approval process with our committee that may take up to two weeks. Their decision is final. There are no appeals.
